Here are some quick links to ServiceOntario resources for students to get an Ontario Photo Card (OPC) and Health Card (HC).

 Ontario Photo Card (OPC)

• Government ID for non-drivers
• Bring original identity documents (e.g., birth certificate) to prove your Legal Name and Date of Birth
• $35 fee
• Valid for 5 years

 Apply for OHIP health insurance and get a Health Card (if you never had one before- such as – if you are new to Canada)

• No charge
• Bring with you:

1.  a completed Registration for Ontario Health Insurance Coverage form
2.  three separate documents from the List of qualifying identification documents (photocopies not accepted) that proves your:

• Canadian citizenship or OHIP-eligible immigration status (original document)
• residency in Ontario (original, printed or digital/electronic document as noted on the list)
• identity (original document)

Renew a Health Card

• Bring with you:

1. your current health card
2. proof of your Ontario residency and personal identity
