Service Ontario
Here are some quick links to ServiceOntario resources for students to get an Ontario Photo Card (OPC) and Health Card (HC).
- Government ID for non-drivers
- Bring original identity documents (e.g., birth certificate) to prove your Legal Name and Date of Birth
- $35 fee
- Valid for 5 years
Apply for OHIP health insurance and get a Health Card (if you never had one before- such as – if you are new to Canada)
- No charge
- Bring with you:
- a completed Registration for Ontario Health Insurance Coverage form
- three separate documents from the List of qualifying identification documents (photocopies not accepted) that proves your:
- Canadian citizenship or OHIP-eligible immigration status (original document)
- residency in Ontario (original, printed or digital/electronic document as noted on the list)
- identity (original document)
- Bring with you:
- your current health card
- proof of your Ontario residency and personal identity
